The application forms are available to be filled out online. Click here to obtain the forms and instruction page. All five pages need to be filled out. Students, please print off the first two pages once you have filled them out and deliver them or mail them to the Rotary Club that will be sponsoring you. The other three pages can be filled out later and mailed to the address given on the bottom of the Medical Clearnance form.
